    Does your child suffer from any allergies (Hayfever, bee stings etc)?

    Has the player played or registered with a club outside of England?


    Volunteering - The club relies entirely on volunteers from within the membership to assist with coaching, officiating, first aid, team management, fundraising, social activities, administration and marketing. Please state in which capacity you would be interested in helping.

    1. All players, officials and supporters understand and accept the FA's respect campaign and codes of conduct and that any person disregarding these will be instructed to leave the tournament immediately. This includes abuse of referees.
    2. All players, officials, and supporters understand, accept, and will abide by the tournament rules and all referee decisions.
    3. It is each team's responsibility to ensure that anyone assisting holds valid safeguarding and emergency aid qualifications and emergency aid appropriate for grassroots youth football.
    4. I will abide by the Club's constitution, club rules, code of conduct, FA initiatives eg Respect Campaign, disciplinary policy, equity policy, social networking, and child welfare policy. I understand that if the above-named member does not also abide by these rules/codes of conduct/policies that the club may take disciplinary action
    5. I certify that the above information is correct and I consent to the information I have provided on this form being used by the Club for any purposes under the GDPR ACT OF 2018. I also certify that I do not have any outstanding liabilities with any other clubs I have previously been registered with. I understand that failure to disclose such information or make any false statement will render the registration invalid and liable to sanction.
